To Start Our Service: Submit your form. We will
set up your account and contact you with your account number and how to access
it online. You will need to have your credit card number and exp. date to activate
your profile. At that point you will be able to choose how many of each product
you want delivered and which months you want delivery of that item. You will be
able to customize each product and change your order as needed. If you have any
questions, or have not heard from us in 2 days with your information, you can
email us.
FILLING
THIS FORM OUT DOES NOT ACTUALLY ORDER ANYTHING! It initiates set up. It will then
be ready for you to access it online. Nothing is ordered until YOU activate your
account! If you want help setting up the amounts, etc., we will be happy to help
you or do it for you! You are under NO obligation by filling this out. You can
decide to do it or not before you activate your file. If you choose not to do
the program, your file will be deleted.
